Preoperative Sedation: IV: 0.05-0.1 mg/kg/dose; repeat q2-3 min prn up to total dose 0.1-0.2 mg/kg
IM: 0.05-0.15 mg/kg 30-60 min before surgery (max 5 mg/dose)
Sedation: 0.1 mg/kg
IV q1-2h prn or loading dose 0.05-0.2 mg/kg
IV, then continuous infusion 0.05-0.12 mg/kg/hr. Titrate as needed.
Oral: 0.2-0.4 mg/kg (max 15 mg) 30-45 minutes prior to procedure.
Intranasal: 0.2 mg/kg x 1 (using 5 mg/mL injectable product in a needleless syringe)
Respiratory
depression, hypotension; reversible with
flumazenil.
