Arterial or Venous Thrombosis or Pulmonary Emboli: IV loading dose 4400
units/kg over 10 min, then 4400 units/kg/hr continuous
IV infusion for 12-72h. Individualize dose based on response; reassess clot q12-24h.
Partially occluded catheter: Continuous
IV infusion of 150-200 units/kg/hr in each lumen for 8-48h
