5-10 yrs: 18 mg/kg/day IV/IM q8h
>10 yrs: 15 mg/kg/day IV/IM q8h
Infuse over 30 minutes. Therapeutic peak levels 20-30 mcg/mL and trough levels <8 mcg/mL. Adjust dose in
renal impairment.
Side effects include
nephrotoxicity and ototoxicity.
